This document contains everything you need to submit your nomination, including: the eligibility criteria, the nomination form, and the report template.

 

Nominations for this award are to be submitted in two sections. Section A, which is a 250 word summary, is due on November 16, 2012. The summaries will then be reviewed by the selection committee and a shortlist of top contenders will be formed. Those shortlisted will be asked to complete Section B of the nomination template and submit it to the College by February 1, 2013. For full details, please download the template.

The Quality of Life Award honours those who go beyond their job description to improve their patients' lives through their desire, creativity and dedication.  It is a celebration of the human spirit.

 

The winner will recieve a one-of-a-kind piece of art signifying outstanding accomplishments, a framed certificate of achievement, paid travel and accommodations to attend the College's Awards Gala evening and one complimentary registration for to attend the National Health Leadership Conference.
